John Windom, 64, a retired Navy officer, was charged with making false statements, concealment of material facts and falsification of a record or document tied to a failure to report more than $15,000 in cash, casino chips and other gifts from contractors. The Justice Department announced the charges on Wednesday, accusing him of “accepting, and sometimes demanding, extravagant gifts from a group of contractors and subcontractors who worked on the project he was overseeing.” As the indictment notes, Windom was aware of the rules regarding reporting gifts and receipts, per training for officials. “An indictment is merely an allegation. All defendants are presumed innocent until proven guilty beyond a reasonable doubt in a court of law,” the Justice Department said in its release. In 2018, the 10-year contract was awarded to Cerner (now known as Oracle Health after Oracle acquired it). Windom was appointed as Executive Director of the Office of Electronic Record Modernization in 2017 and helped supervise the VA’s choice of Cerner for the contract. According to the grand jury indictment against him, Windom used his position to take money and gifts from a group of business executives from information technology and consulting companies who were contractors on the project. According to the indictment, Windom allegedly called them “the Power Group.” Windom used his role to “encourage, monitor, and facilitate contract and subcontracting opportunities for members of the Power Group, related and unrelated to the EHRM project.” Windom was allegedly given payment in everything from cash, to $8,200 in Louis Vuitton gift cards and $1,800 in casino chips. He was also given a High Efficiency Particulate Air, or HEPA, filter worth $631. The indictment also accuses Windom of using his position to “coerce” payments and gifts from others. Per the indictment, Windom “repeatedly reminded Power Group members to remain loyal and directed them to maintain confidentiality and their relationship with him.” In one message at the start of 2020, Windom messaged Power Group members, telling them that “loose lips sink ships.” Windom’s initial contract was extended, and in 2022 he was reassigned to serve as deputy director of the Federal electronic Health Modernization Office, a joint VA and Department of Defense project. Nine years after the modernization effort started, the Electronic Health Record program is used by only a handful of VA sites, with the program hit by glitches and usability issues. The VA paused the program in 2023, but is expected to roll out at several sites this year. Windom faces a maximum sentence of over 20 years in prison if convicted on all counts. Dr. Mulki is a very competent doctor. His office/billing staff is HORRENDOUS, so I cannot recommend his practice. More than a YEAR after I started treatment for a Workman's Comp case, I was told they had NO IDEA it was Workman's Comp, so it was all MY responsibility and it was going into collections. I had responded to ALL bills with the COMPETE workman's compensation paperwork, made special visits to the Dr's office to give them updated codes and so forth, and they (suddenly!) said I had never done ANY of that. Thankfully I work for a government agency, so I can pass this onto them and they WILL sort it out, but I'd steer clear of this office until enough reviews show that they've gotten their act together!

<b>BEWARE OF THIS UNSKILLED AMATEUR!!</b> I would <b>NOT</b> recommend this "doctor" to anyone. I had great pain in <i>two</i> teeth and she insisted on pulling <i>three</i>! Kellman did get them pulled (all three) but, she had trouble pulling them <i>straight</i> out!! She REFUSED to prescribe antibiotics or pain killers and I had a very tough night with a swollen face this AM. Kellman told me to take pills I had told her I cannot take. Kellman has told the state of Illinois she is an anesthesiologist in Burlington, IA but, she is practicing as a dentist in Oquaka, IL. And not a very good one, either! Is she really a doctor?? I think NOT!!

Ordered medium cajun spice Received “FIRE HOT” to the point I had tears running down my face & Throat was on fire for hours. I called and let them know they made a mistake. The manager offered to remake my order, I went back next day picked up shrimp, corn & potatoes at no charge. Keep in mind I brought back what I couldn’t eat to show them I was being honest! At this point I wouldn’t recommend this establishment to any one. To add insult to injury on top of all the mix Up after checking my online banking, They took it upon themselves to add a $9 tip. My Total was $46.31. I’m Not A Happy Camper..

I got food poisoning from their food truck at the Texas State Fair. Deep Fried Froot Loops and Deep Fried Yam. So many complaints against them they should be shut down for the health and safety of all Texans.

This is the worst Taco Bell! EVERY time we order a Beef crunchy taco sup. They give us a reg. Crunchy taco. This visit I ordered 3 chicken chalupa Sup., the shell was so hard I didn’t even eat it, our taco sup. Was a reg. Taco broke in half because they stacked the 3 chicken chalupas on top of it. I also ordered a 12 pack. Cinn. Treats. They were ok. I should have thrown a 20$ bill in the toilet and flushes it. Out of all the food I ate 2 Cinnabon things. They just now started wearing gloves and masks and handing the card thing out of the window so I can swipe it myself. All other Taco Bell stores started this when the Pandemic began, it’s now Sept. and they started doing it at the end of Aug. Corporate really needs to visit this store and stay a week and watch. Go thru the drive through unannounced and see what you get. This store is ran by a bunch of teens and very young adults that should not be in charge. If they can’t find responsible managers to manage the teens then close the store down. Food is always wrong and terrible. I am done with them. I will go to the Taco Bell on Post Rd off of I-74 from now on! That store is awesome...just a little farther, which is ok with me!!!

Poor judgement and lack of responsibility by WCCO to feature this restaurant's Smack Shack Festival with its monstrous crawfish boil: Their representative openly confessed that they netted the crawfish from Woman Lake to near-extinction. (Crawfish are an important part of the eco-system of a lake!) Shame on them and SHAME on WCCO... I would not recommend attendance anytime, let alone during an epidemic! FACT: Festivals = Spread of the Covid virus! Help us lower the risk to all by DISCOURAGING such public nonsense!

Hostile and unprofessional work environment. Anthony, one of the many “owners” who owns about 1% of the business will scream at you in front of guests and blame it on his “personality”. Peter, one of the many “owners” will mess up your checks, not catch his own mistakes, and when you come to him with a problem, will literally nod off while you’re talking to him. He will also light up a cigarette in your interview, like he thinks we are in Paris or something. All owners are unbelievably unrefined, unprofessional, and just prove that any one can go into business for themselves if they have the funds. Doesn’t make them real worhy professionals though. Checks were late, was screamed at in front of customers, checks were not accurate, payroll messed up many times, and the work environment and restaurant is rudimentary and lackluster, but everyone you work with will think they’re curing cancer. Don’t work here. Go work for someone where you will actually be paid on time and correctly and not abused.
