I was getting screened by the nurse. She asked about my medical history along with the standard questions, have I been exposed to Covid, etc. I told her I had Covid at the end of Nov. She then asked what brought me in today. I told her but she kindly interrupted and said, "you just need a referral, correct." I said yes. I said, I'd also like some blood work done while I'm here, nothing of concern just might as well get a physical while I'm here. I also mentioned my throat was irritating me but I also get chronic tonsillitis at least once a year. I told her it wasn't anything I was really concerned about, but hopefully I could talk to the doc and possibly get a z pack. She said, ok let me go talk to the Doctor. He immediately came in, and read me my rights so to speak. I was actually fearful when he stepped foot in the room. His look he gave me was petrifying. I was appalled at his dismissive, assumptious meanness towards me. He right away said, "why would you ever come into this office with Covid symptoms." I said, I don't have Covid symptoms, what are you talking about. He said you have a sore throat and how dare you come into this office when you're sick. He said, what are you even in here for in the first place? I said, I came in here for a referral and thought it was a good idea to get a physical and mentioned my throat hurt. I said I had Covid the end of November and there's a 90 day period where you are free from reinfection. He was so mean. Not to mention all this took place in front of my five year old daughter. Even my daughter was scared. I said, I know it's not Covid its just a sore throat and you can't automatically just because my throat hurts, it's Covid. He said, well what else could it be then. I was shocked. I said maybe it tonsillitis. Then he said, how would you even get that then. I said, I don't know, germs... It was just back n forth banter. He then came back in a hazmat suit and said it was in my best interest to get Covid swab and Strep throat swab. He came back in 10 min later and cleared me. It was the most pointless waste of time/energy appointment with someone who has zero compassion for anyone and who made me feel like I was this disgusting walking germ in front of my daughter. Never want to see his face again!!!!!

The experience I had with some restaurant employees was really terrible. Breakfast is supposed to start serving at 4 but I went to pick up an order at 3:30 and they refused to serve it.When I asked the boy why? If breakfast starts at 4, one girl aswer me very rude and disrespectful, YELLING that I had to move and cancel the order (I'm Uber) I told her to calm down, stop yelling me, those weren't manners to talk to a costumer, and I asked her for a complaint form. She told me go to window. After waiting for almost half an hour, another girl came out and gave me Uber cancellation number. I asked for the claim sheet and she told me they didn't have it, she turned her back on me and left. When I asked for the manager, a Latin Lady told me that it was her, I asked her for her employee id and she told that she didn't have one, the manager wasn't there. Another Latin boy replied "the manager had more important things to do than take care of you ". And finally a very young looking american white boy (or girl) came out without id, saying that it was named Mikey, who was the manager, and could not give me the name of the disrespectful employee, He / she TREATED ME TERRIBLE, and asked me to leave ... Complainant S.S. was present telephonically and made a statement. She initially had a doctor-patient relationship with the Respondent in 2007, but after six (6) months it turned ***ual. S.S. stated that her daughter never made an appointment and she doesn’t know where the Respondents new practice is. The Board asked S.S. when she stopped having ***ual relations with Respondent; she stated it was July 2017. Complainant M.P. was present and sworn in by the court reporter. M.P. would like to testify against the Respondent for inappropriate ***ual misconduct with her. Respondent took advantage of her when she was in an unstable period in her life. After experiencing several episodes of ***ual misconduct with the Respondent she now has fear and trauma. She felt obligated to give ***ual favors within the clinic on several occasions. She began seeing Respondent in November 2011 through August 2017. During an appointment she stated the Respondent would ask her if she wears granny panties. He then suggested she start wearing thongs. She states that the Respondent stalked her and made inappropriate remarks to her family. M.P. reported to the police of the Respondent stalking her. M.P. is asking the Board to surrender the Respondents license as a physician. The Board had a discussion with M.P. asking when she first came in contact with S.S.; she stated April 2018 through Facebook. M.P. stated that Respondent was able to remove her student loans of $135,000 by entering in her chart notes that she was mentally retarded. Respondent admitted to having oral *** with one patient outside of the office to the Investigator during an interview. Respondent stated he had ***ual contact with patient S.S. in July 2017 but only one time. The Respondent stated he never had ***ual contact with M.P. Respondent stated that he did show S.S. and M.P. his nipple rings. Dr. Reiss moved to enter Executive Session for legal advice. Dr. Maitem seconded. The Board entered Executive Session at 5:26 p.m. and returned to Open Session at 5:40 p.m. The Board asked M.P. if Respondent ever displayed his penis to her, she stated yes in April 2015 after he gave her a massage and while she was filling out her disability papers. He took his pants down and she gave him oral ***. He had *** with her many times in the clinic on Thunderbird. Respondent knew he had boundary issues and took a boundary course in October 2017. Prior to the allegations he used to hug patients and now he does not because it can be misconstrued. Dr. Cunningham moved to enter Executive Session to review medical records. Dr. Reiss seconded. The Board entered Executive Session at 5:51 p.m. and returned to Open Session at 6:07 p.m. S.S. stated that she contacted Honor Health to make an appointment with the nurse practioner, whom took over the Respondents patients. She did not want to stop all medications that Respondent had been prescribing to her. Dr. Cunningham moved to enter Executive Session for legal advice. Dr. Spiekerman seconded. The Board entered Executive Session at 6:12 p.m. and returned to Open Session at 6:17 p.m. MOTION: Mr. Landau moved to refer this case to the office of Administrative Hearings for a formal hearing on Revocation of license and this is based on violation of ARS 32-1854 (6), (20), (38), (42) & (45) and incorporate the reports by the Medical Consultant and the Investigator and specifically state that the Respondent committed ***ual conduct and contact with a patient, exposed chest piercings to a patient, admitted ***ual conduct with a patient, medical records were incomplete, prescribed ***s and benzodiazepines at the same time. I sincerely thought I would be giving this company a rating if “5” after I worked for them since 2014 up until 2021. Sadly, after 7 devoted years of working my way up from being a walk in hired manager who co-managed with multiple other managers at different locations to being given my own wash that I ran for over 2 years without any major problems up until the pandemic started. For some reason we (a car wash) was labeled as “essential”. This did ensure my employment during a very hard time while others were finding themselves unemployed however, my household faced a problem which caused me GREAT emotional, physical and mental hardships - when Covid broke out, my (at the time) 9 year old stepsons doctor contacted my wife and informed her that she would need to lockdown our home to ensure that he didn’t come in contact with this virus due to the fact that he had a low immune disorder and when people did have to enter her home she needed to take all precautions possible. And there I was, having to accept the fact that my working to ensure my families home and financial well-being and being blessed to be able to continue to do so however I found in doing so and returning to our home each day put my stepsons LIFE at risk. I didn’t realize that this internal war I was having within my head was reflecting in my work. Rather than calling me in, sitting me down and talking to me, asking what’s going on, my higher-ups chose to use my few mistakes- and YES, there were literally 2-3 mistakes I made right before my boss who I answer to came to the wash I ran and literally took my shop from me, no questions asked about my mistakes. He stripped me of my head manager title and relocated me to a wash that was approx a 45 min to an hour drive away from my home and my family which was literally smack in the middle of a “high rating Covid zone”. I took this demotion as I was not going to flat out quit due to knowing that finding another job during this time would be near impossible. For about 3-4 months I went out there to work. Within the first week I realized they had canceled my company gas card which I seriously needed in order to continue to have reliable transportation to the newly demoted position. The “head manager” of the location treated me like I was new and this was literally after 7 years of devotion to this company. One day I was late, I called on my way in while stuck in traffic on interstate letting them know I would be there as soon as possible but I was at a stand still sue to a wreck ahead of me. That day nothing was said to me about anything however the next morning when I arrived at work on time, I was called into the head managers office and I was FIRED on the spot! I LITERALLY WAS NEVER THANKED FOR ALL THE TIMES I WAS THE REASON WHY THIS MAN RONNIES BUSINESS LOCATIONS WEREN’T CLOSED DUE TO BROKEN PARTS OF THE WASH - I was way more than just a manager - I WAS THE CAR WASH MECHANIC & THE AUYOMOTIVE MECHANIC bc when the wash damaged a customers vehicle, I was the person who fixed it- not a shop ( unless it was needing to be painted). Needless to say, THE PELICAN POINTE CAR WASH located in SOUTHEASTERN LOUISIANA AREA TREATED ME LIKE TRASH AFTER I FULLY DEVOTED MY LIVE BESIDES MY HOME LIFE TO WORKING FOR THEM - I even had planned to retire from that company bc I did enjoy my time employed there ... even without praise, thanks or acknowledgement... even after doing good things which benefited the company they chose to degrade anything they chose to rather than admit that on MANY different occasions, I was the ONLY one who was worth a damn. I have RECEIPTS for everything I’ve stated- EMPLOYEE WITNESSES as well as PERSONAL WITNESSES of the way I was treated... As the owner celebrates 10 years of 2 *** shows working got him, he could care less about everything I did - I admitted then AND am saying it now - I WAS MARRIED TO MY JOB... got treated like absolute *** and I honestly REGRET every penny I saved that USING SORRY EXCUSE FOR AN “EMPLOYER”! And YES, I AM in contact with lawyers to discuss possible legal ill-will they caused me due to their treatment of me and their refusal to concern themselves with my well-being during a literal pandemic... DISGUSTING AND UNFORGIVABLE!

MJP Family dental has been trying to collect money from us for a cleaning they never did. We don't even know this dentist. His name is Dr Katz. They have been harassing us for payment for a service they didn't do. A previous dentist that used to own the business did the cleaning. MJP Family dental coded the claim incorrectly and our insurance rejected the claim. MJP Family Dental would not resubmit the claim to our insurance company. They sent us to Collections because we wouldn't pay the bill. We are not paying it because it was covered 100% under our insurance. They refuse to resubmit the claim with the corrected TAX ID number. Yes we have been in contact with them but they just want money from us not the insurance company. And then they continue to call us to pay this bill after they turned it over to Collections. Carmen Epstein called after this account was turned over to the Collection Agency. That is not even allowed!! Pretty sad that they feel the need to do that to older people. We ALWAYS paid our bills. Never been turned over to Collections. This was in August of 2018. It's now 2 And a half years later and they need to get it together! Something not right is going on!

We are dismissing you bcuz you didn't show up after I told her that I could not walk. N. Had. No way there. Earlier that morning. Well. I thought my dr was going to get on the phone. my appt was at. 2.40 PM. Instead. Ashley a nurse sd. good luck with your car shopping and the bank. Good luck with your life. I was shocked when she said that. n. My older brother had just came by then. To hear what was said over the phone and he said that was crazy. Yes it was n. Mean. I'm disabled due to back problems and neck problems 5 bulging discs and deg disc disease and rheumatoid arthritis and Fibromyalgia. N. Sciatica nerve problems are around a disc. Got. No. Referral to a specialist or any other medical professional advice from them. Just. Good luck with your life and I said thank you. I have been told to call the AMA. N. Medical board on this type of treatment. But before that. He said he knew that I hurt. He. Had the. New. MRI. N. I only met him once and the other appt was by phone bcuz of covid. I have nothing against the desk girl they are doing as they are told by the psychiatrist who does not even has a SPECK of professionalism. I went there at a very low time in my life in august of 2019 and all she did was ramble one and one and one as i am mentally unstable and unwell. She is VERY VERY VERY hard to understand. she asked you a bunch of questions doesn't even give time for you to speak. so all my yes and no's have no explanation of my experience or my current mindset. I got sent home with medication and every month i had to pay for a visit and pay $100. Which we are still going back and forth with the insurance company about because this lady told my mom SHE DOESN'T NOT WHAT THE INSURANCE TO PAY HER BECAUSE IT WILL TAKE TO LONG. SHE IS A FRAUD!!!!!!! ONLY WANTS THE MONEY!!! Since i needed care i paid it. February 2019 all of a sudden i was being charged $20 - and i was shocked and asked the front desk girl why? She said "YOU MET YOUR PATIENT DEDUCTIBLE". I said great awesome - went home with $80. MARCH 16TH I GO AGAIN - DIFFERENT GIRL AT THE DESK - NOW I OWE $180. I was flabbergasted as to WHY?!?!?! Why they said I didN'T meet my deductible. Maybe Dr. Wang was able to pay her rent in February but YOU DO NOT PLAY WITH PEOPLE MONEY LIKE THAT. I asked for my medical records to be released to be able to leave her care. Now it is June 1st and she refuses to release my Paperwork due to the amount i owe. Used the McDonalds app Deals for 50% off double cheese burger. The cashier at the drive up scanned the coupon on my phone, but it was not for my order. Possibly it was for another customer from a second drive up line. When she then pulled up my order, she rescanned the coupon, but said it wouldn’t work. It was because it was used already. Yes, on the mistaken order it was just applied to a minute earlier. So she said I couldn’t use the coupon and I would have to pay full price. I said to her that’s not right. She said there was nothing she could do and that if I wanted my order, I would have pay without the benefit of the coupon. There was a line of customers behind me, so I just paid and took my order. I still feel this is not right.

I had not been to a Dentist in decades. Like many others I dreaded the prospect of going and luckily had no pain until it finally happened, I took a bite of food with a hidden bone inside and broke a tooth. The throbbing pain wouldn't stop so I called a friend for a referral and they could not stop, or slow down, the amount of amazing praise for Dr. Murat Atli. They went on about how "Lucky" I was and how much "Fun" I would have and that I was in for a "Great Experience". So at this point I'm thinking I've got to remember to have my friend looked at for mental health issues as soon as I get rid of the excruciating pain I was in. The tooth was so shattered it needed to be extracted piece by piece. Not normally an event one looks forward to experiencing however my friend wasn't looking so crazy as Dr. Atli's bedside manor was truly refreshing. In this age of go fast next patient, now next hurry hurry he took time to alleviate my fears and explain all that would occur and the changes since I'd last seen a Dentist. The process was truly remarkable, I did not feel shots to numb the area nor the stitches placed once removed. I felt only the pulling sensation when removing but No Pain, simply Unbelievable. Dr. Atli was Professional, Accommodating, Enlightening Caring and yes Fun. A Wicked sense of humor he definitely has. I now go out of my way to reccomend to friends, family even strangers the Amazing Dr. Atli. I've handed a business card (I keep some with me crazy as it sounds.) and even hand introduced people to this Brilliant, Talented and Very Rare Special Man on numerous occasions. I have never had a bad experience with and I Highly Recommend Dr. Murat Atli ! ! !

Mrs. Hevener English 111-05, she is the worst teacher you could get. She has her favorites. I've worked hard on all my papers and made sure it was the best it could be but still got a grade I shouldn't have gotten, but would give someone who's paper wasn't that good with bad grammar gave them a better grade then they should have gotten. I would not recommend not to take her class yes it is easy but if she doesn't like your paper or your not her favorite then you will not get the grade you would want.

Janet came to our company and department with the idea that we were all inept and that we needed to be trained on introductory project management. Yes, I agree that we needed more process and sophisticated tooling in place, but she completely disregarded the fact that we are professionals all coming from different backgrounds. We actually harnessed a lot of expertise in this area, but she didn't explore that. She offended the group by spending almost the entire area meeting giving us "lessons" on agile development, project management, and Microsoft Project. We were all frustrated with her lack of consideration for the group. She also dictated a group project that is usually decided as a group effort. The project revolves around setting up folders on a network drive for project documentation, which completely ignores any type of well thought content strategy. She is bossy and seems to only care about what the executives think about her. If she is ever your manager at CO-OP or any other company, I would run as fast I could. She doesn't care what you know - it's all about her.

She expects students to understand every instruction she gives, and only regrades assignments for 6th graders only. Yes, 7th and 8th graders need less help, but they still need assistance. One time, she changed the due date to an earlier time and expected students to turn in the assignment at that specific time. When students told her that there were some technological issues and the assignment was turned in a bit later, she commented back: “LOL. You procrastinated. If I had made the due date 6:00, you would’ve told me at 6:01.” You would’ve thought that as a tech teacher, she would be most understanding about tech issues. But no.... Students are afraid to ask her questions to clarify because of her attitudes. Many students dread going to her classes. What should I say? “LOL”?

when Doctor Nelson arrived, 10 minutes late, He shook my hand (about 5 inches from my body) and said," What are you here for." I had, of course, told the nurse and her notes were on the computer screen in front of the doctor. I started to say I was having urinary symptoms when he started to read my concerns. He said, "I see you have nocturia, and weak stream anything else?" I said, "yes I have the symptoms listed for BPH. What are those other symptoms?" He said, " No, that's not how this works. You tell me what your symptoms are." I I said," Ok, I'll look them up on my phone." He then stood up and said, "I'll be back when you know what you're talking about." He then left the room. He returned about 7 minutes later and said, "are you ready to talk now?" A bizarre thing to say since I was talking when he left the room. I showed him the list of symptoms listed for BPH and said, " Listen, you must understand I'm very embarrassed and frightened to be here. I didn't want to forget anything because of that." Dr. Nelson was looking at the ground and repeating yeah, huh hu, yeah. I said, "oh wow, you're not listening to me." He said, "I don't think this is going to work out." I got my coat and walked out of the office with Dr. Nelson yelling as I left the office.
