Dr. John C. Amann, M.D, F.A.C.S practice is Neurology and Neurosurgery Associate, P.A. in Winter Haven. FL. I went to Dr. Amann for a herniated disk in lower back. First visit saw P.A. said I would need to see Dr. Amann. My visit with Dr. Amann was to explain what he would do during surgery. He advised if given the shot it would be a 50/50 fix but with surgery it would 90% for complete recovery. I chose to have the surgery and at this time he sat down with us and answered any question my wife and I had. The morning of surgery I arrived at the hospital and checked in. Surgery was to be at 7:30. He arrived early and took me in to surgery at 6:50. I was able to leave by 10:00 to go home and recoup. Dr. Amann called that afternoon to check on me to make sure all was good. I returned to his office two weeks later for a follow up. Did not even sit down in waiting room and was taken back to his office. Nurse check all vitals and walked out of room and Dr. Amann came in within two minutes. He looked at the incision and said it was healing nicely, sat down and talked with us about what he had done during surgery and what to expect for the healing time. He asked if I had any questions and he sat and answered all questions. I saw Dr. Amann once before surgery, day of Surgery and the follow up visit. Not one time did I have to wait to see him. Very courteous and straight to the point. I would recommend him for anyone needing spinal care.

I was lucky to find Dr. Nass. He has expertise, and is board certified, in nephrology and internal medicine. He is up-to-date, thorough, caring, dependable, responsive, clearly communicates test results, answers all my questions, and his treatment plans work. He is the epitome of a great doctor. Because of changes in my medical plans, I've had several nephrologists and he is the very best, and among the very few best of any specialist and PCP I've had. He cares for, and treats, a range of health needs, his testing is the most thorough of any of my prior doctors. He changed my treatment, and changed prior medications, so I had better outcomes. He's concerned about all my health issues and his internal medicine expertise is a definite plus when dealing with my treatment. I recommend him highly as one of the very best in his field.

Dr. Michael Sullivan, Gaylord, MI
Very sad experience. Dr. Sullivan performed a root canal on my number 13 molar. The root canals of this tooth are very narrow and extended. I warned the Dr. that I had a traumatic experience with elongated tooth roots which for teeth being extracted both as an adolescent and as a young serviceman in the Navy. Both of those times the dentists had a rough time with the teeth. The procedures lasted over 4 hours for the tooth when I was a child, and two visits over 3 hours each when I was in the Navy. Dr. Sullivan assured me that he was capable, and that his skill was superior. I believed him. After his first attempt at root canals on this tooth, Dr. Sullivan sent me home telling me that there wasn’t enough time that day to finish the procedure. Mind you I’m under the influence of sedation at this time. I told Dr. Sullivan that I would rather have the tooth pulled than go through another 3 hours in the chair. Amazingly I believed him again when he told me the reason he couldn’t finish was because of scheduling. He also said that he wanted to prove to his dental assistants he could finish the job! Being as high as I was on the anesthesia I made the poor judgement call to return. Two days went by with excruciating pain in my upper jaw. I was having to take Tylenol and Advil to relieve the discomfort. I went back to Sullivan’s office, asked AGAIN if it would be better to just extract the tooth, and he said it was his opinion he could save it, and this time he could finish the job and apply the cap. So I strapped on his gas mask and knocked myself stupid only to have the “Doctor” root around, stretch my jaw and break off half of what was left of the tooth. When I came to I distinctly remembered the snap, the dental assistant saying “Oh God!”, and the good Dr. scurrying away into another operatory. What was I told this time? Of course that I’d have to come back since it was the tooth’s fault it was so damaged that it cracked, and now he had to make a new cap with his machine. Stoned on the good Dr.’s laughing gas I told him “that’s okay, mistakes happen”. What an idiot I was. The laughing gas wore off and the Tylenol Advil routine lasted a week. I went back to Beacon Dental Center, this time for the last time, to finally get the remnants of my tooth capped. This time he squirted the glue in to fill the root canals, fixed the cap on the tooth, and away I went. Fast forward 5 years and I’m in the chair of a very competent dentist. The dentist asks if I’m in any pain and I mention the constant pain I’ve had on my left sinus area. I’ve felt this pain for three years. I’m shown the current X-ray of the area and the Dentist tells me that it’s not normal. The capped tooth is great work (machine made, remember) but there is stuff in the canals of Number 13 tooth that extend into the sinus. After visits to an Ear Nose Throat MD, Oral Surgeon and Endodontist specialists, I’m scheduled to have an operation to remove the so-called saved tooth, and resultant fistula created by the extensive overfilling of the Number 13 root canals with guttapurcha, the latex based root canal filling material. Predictably I could sue and win to recover the costs of these procedures, but the good Doctor has closed his practice, and is now enrolled in a school in Detroit. The lawyer I consulted doesn’t see that my case is strong enough to show the pain I’ve been living through since I trusted Dr. Sullivan. Of course there are risks to root canals. I accepted the possibility of a dry socket, and possible infection, but was re-assured by Sullivan that he’s done thousands of these procedures with no bad outcomes. General dentists can perform root canal procedures along with other dental procedures, but often they refer patients needing endodontic treatment to a specialized practice, who works in collaboration with your dentist. An endodontist is a dentist who specializes in saving teeth. Dr. Sullivan should have not touched that tooth after I warned him of the history of my other root issues, and certainly after he saw the roots narrowness and length.

I was getting screened by the nurse. She asked about my medical history along with the standard questions, have I been exposed to Covid, etc. I told her I had Covid at the end of Nov. She then asked what brought me in today. I told her but she kindly interrupted and said, "you just need a referral, correct." I said yes. I said, I'd also like some blood work done while I'm here, nothing of concern just might as well get a physical while I'm here. I also mentioned my throat was irritating me but I also get chronic tonsillitis at least once a year. I told her it wasn't anything I was really concerned about, but hopefully I could talk to the doc and possibly get a z pack. She said, ok let me go talk to the Doctor. He immediately came in, and read me my rights so to speak. I was actually fearful when he stepped foot in the room. His look he gave me was petrifying. I was appalled at his dismissive, assumptious meanness towards me. He right away said, "why would you ever come into this office with Covid symptoms." I said, I don't have Covid symptoms, what are you talking about. He said you have a sore throat and how dare you come into this office when you're sick. He said, what are you even in here for in the first place? I said, I came in here for a referral and thought it was a good idea to get a physical and mentioned my throat hurt. I said I had Covid the end of November and there's a 90 day period where you are free from reinfection. He was so mean. Not to mention all this took place in front of my five year old daughter. Even my daughter was scared. I said, I know it's not Covid its just a sore throat and you can't automatically just because my throat hurts, it's Covid. He said, well what else could it be then. I was shocked. I said maybe it tonsillitis. Then he said, how would you even get that then. I said, I don't know, germs... It was just back n forth banter. He then came back in a hazmat suit and said it was in my best interest to get Covid swab and Strep throat swab. He came back in 10 min later and cleared me. It was the most pointless waste of time/energy appointment with someone who has zero compassion for anyone and who made me feel like I was this disgusting walking germ in front of my daughter. Never want to see his face again!!!!!

Wheeler & all 3 owning Drs. after 10+ yrs. of care, just dropped me b/C her receptionist of 20 yrs lied when I explained to her the steps I was ordered to follow by my Dr. to protect me from CV-19 exposure b/c MI Law, required me to personally pick up a script at the Dr. office.Receptionist didn't like to deviate normal protocol, thus retaliated against me & lied. No disrespect etc. on my part. The Dr. dropped me & never investigated or fired receptionist for lying. No other reason. This same incident happened twice--again at time of next refill. Unfortunately I didn't report 1st incident at the time.One of their own patients was a witness.I have no recourse but to lose my Dr. I'm 65 years old & the Drs. policy is if a patient is dropped, no other Dr. in the huge practice will see you--blackballed. I know they didn't investigate b/c I had witnesses both issues. She/they never tried to contact me--just mailed a letter. Office Mngr believed.She offered to put me with a Nurse Practitioner. I have complex med. issues & need someone with more extensive knowledge & practice time. Office itself has issues otherwise.Can't recommend. They were 100%UNPROFESSIONAL, INHUMANE, UNETHICAL when most needed. My doctor knows me better than to have believed I would do anything out-of-line or disrespectful. All this & NO DOCTOR during a worldwide Pandemic. Unbelievable from beginning to end of this nightmare. After all these years they didn't give me the common curtsey to ask me what happened, as ‘their patient’. I never owed even $1 to the practice—ever. The 3 Dr.s that own the practice are also aware there is ‘a situation’, but know nothing of the truth. They won't take time to learn the truth or deal with their responsibIlity. They need to be reminded that their first obligation to their patients is to 'Do No Harm...' It never crossed my mind that they would need to be taught to 'CAUSE No Harm'. PATIENTS BEWARE.

First visit I had with him he told me I had lots of options to deal with my hearing problem. Second visit with him he installs a drain tube in my ear. Third visit with him I tell him I didn't think it was working (a month and a half later). He says OK then proceeds to pull out the plug, he gives me no explanations, no options, no nothing. Needless to say it hurt pretty bad and his silence made me wonder what on earth is going on. He then walks out. His nurse who is attending him helps me stand up then notices I have blood draining out of my ear onto my face down my neck and on to my shirt she gets several kleenex tissues and cleans me up then ***s me to the front desk. As I'm walking down the hallway to the front their sits Doc Kuhn sitting in a room on the left of the hallway reading a paper and smoking a cigar. This is literally and exactly what he did. No explanations, no goodbye, no see ya later, nothing. He didn't even say whether I was to come back or not. I now think he knew all along that he could do nothing for me so he played me for what he could get out of me. My opinion, he is at best disingenuous and at worst dishonest. Also, He is not forthcoming with anything unless you pump him for answers. His general attitude was that I was bothering him. I do not recommend him.

If I could give ten (10) stars to Doctor Barry Boden I would have gladly do so. I have Called several Orthopedic Doctors due to crucial pain on my neck back and right shoulder and arm to get an emergency appointment with no success I called Doctors Boben's office at 20500 Seneca Meadows Pkwy Germantown Maryland a soft spoken stuff member placed me in a brief hold and was able to get me in to see Doctor Boden. The pain was unbearable. I was taken to his office and was greeted by a wonderful nurse that took me to the ex-ray room , a warm and well mannered ex-ray technician took ex-rays and walked me to the examination room. In a few minutes Doctor Boden came in looked though my medical history in detail examined me and listen very patiently to my complains. The Doctor explained the reason of my pain and the symptoms answered every question I had. His unlimited medical experience and diagnosis stunned me. He gave me a couple of shots comfort me with his kind words and scheduled me for a follow up in a week, he asked me to call him if the pain continued , after making sure that all my questions were answered he ***ed me out. His bedside manners and concern about my health condition had stunned me. I have seen a lot of Doctors in the past fifty years but I have never experienced this genuine concern and good care that I have received from this "Miracle" Doctor and his stuff. Now I am pain free and blessed with a great Doctor. I recommend Doctor Boden and his staff very highly to any one that is seeking for a great Orthopedic Doctor. I have been trying to figure out my diabetes for years. At first, I believed she was a great doctor, but as I'm wanting different options, I'm starting to doubt her. What she says in person versus what she writes in clinical notes don't match. She doesn't accurately portray my desires and concerns in her notes either, which makes me seem like a stupid, uneducated patient. She writes that she suggested a treatment but never mentioned said treatment to me verbally. She failed to list reasons for requesting medication changes. It feels like a Dr. Jekyll and Mr. Hyde situation. I've considered recording conversations so that future doctors will know the truth versus what's in my records. I have messaged her and her supervising MD regarding my concerns. If they are addressed properly, I will gladly change my rating and review. If not, I will be finding a new primary care doctor for both my daughter and I.

After 7 years of care, my daughter's psychiatrist retired. We had the misfortune of selecting Dr. M. Abraham at Aurora Behavioral. She admitted to NOT HAVING THE TIME TO READ HER FILE BEFORE OUR VISIT! She then accused me of regularly abusing my child!! She put her in the hospital to "separate" us! (Maybe Trump is her hero?) While in the waiting room, a public location, she came in & stated I should expect a visit from CPS. Fortunately, the MD. in the hospital expressed it is clear there is NO abuse!!!!

I went in for pain in the abdomen and difficulty breathing. My general MD was not available, so I was referred to Dr. D. Carlson at the Great River Family Practice Center in West Burlington, IA. I was asked and responded 3 times to questions regading my allergies. Despite that, I was given a shot and a medication order to be filled at the pharmacy. When I went to the pharmacy to get the medication order filled, I was told "it was not the right medication for me." They would have to call the doctor for another medication. That could only mean, I was allergic to the medication ordered. That is a flagrant disregard for a patient's safety.

I especially appreciate receiving a copy of his correspondence with my primary care physician summarizing what he did and why, plus his overall impression w/recommendations for my on going care. I was taken to the hospital while I was having a stroke. My treatment from the time I arrived at the hospital until I left there was excellent. Dr Mojumder explained everything to me so I could understand it. He had pictures from the MRI and told me exactly what I was looking at. He answered any questions and did not rush through them. I felt very relaxed and confident with him. I think I was blessed to have him as my doctor.

When this pandemic started, no one was prepared for how our daily normal lives would change. Dr. Jason Pirozzolo and his staff at OHSA have been incredibly helpful! I had a telemedicine appointment with Dr. Pirozzolo and he made me feel like I was having an in person meeting with him. He was informative and really cared about the pain I was experiencing. After speaking for him for a while, he had me speak with his assistant to schedule an appointment. I'm doing my daily light exercises until my scheduled appointment and already feeling less pain and more at ease. Highly recommend!

Complainant S.S. was present telephonically and made a statement. She initially had a doctor-patient relationship with the Respondent in 2007, but after six (6) months it turned ***ual. S.S. stated that her daughter never made an appointment and she doesn’t know where the Respondents new practice is. The Board asked S.S. when she stopped having ***ual relations with Respondent; she stated it was July 2017. Complainant M.P. was present and sworn in by the court reporter. M.P. would like to testify against the Respondent for inappropriate ***ual misconduct with her. Respondent took advantage of her when she was in an unstable period in her life. After experiencing several episodes of ***ual misconduct with the Respondent she now has fear and trauma. She felt obligated to give ***ual favors within the clinic on several occasions. She began seeing Respondent in November 2011 through August 2017. During an appointment she stated the Respondent would ask her if she wears granny panties. He then suggested she start wearing thongs. She states that the Respondent stalked her and made inappropriate remarks to her family. M.P. reported to the police of the Respondent stalking her. M.P. is asking the Board to surrender the Respondents license as a physician. The Board had a discussion with M.P. asking when she first came in contact with S.S.; she stated April 2018 through Facebook. M.P. stated that Respondent was able to remove her student loans of $135,000 by entering in her chart notes that she was mentally retarded. Respondent admitted to having oral *** with one patient outside of the office to the Investigator during an interview. Respondent stated he had ***ual contact with patient S.S. in July 2017 but only one time. The Respondent stated he never had ***ual contact with M.P. Respondent stated that he did show S.S. and M.P. his nipple rings. Dr. Reiss moved to enter Executive Session for legal advice. Dr. Maitem seconded. The Board entered Executive Session at 5:26 p.m. and returned to Open Session at 5:40 p.m. The Board asked M.P. if Respondent ever displayed his *** to her, she stated yes in April 2015 after he gave her a massage and while she was filling out her disability papers. He took his pants down and she gave him oral ***. He had *** with her many times in the clinic on Thunderbird. Respondent knew he had boundary issues and took a boundary course in October 2017. Prior to the allegations he used to hug patients and now he does not because it can be misconstrued. Dr. Cunningham moved to enter Executive Session to review medical records. Dr. Reiss seconded. The Board entered Executive Session at 5:51 p.m. and returned to Open Session at 6:07 p.m. S.S. stated that she contacted Honor Health to make an appointment with the nurse practioner, whom took over the Respondents patients. She did not want to stop all medications that Respondent had been prescribing to her. Dr. Cunningham moved to enter Executive Session for legal advice. Dr. Spiekerman seconded. The Board entered Executive Session at 6:12 p.m. and returned to Open Session at 6:17 p.m. MOTION: Mr. Landau moved to refer this case to the office of Administrative Hearings for a formal hearing on Revocation of license and this is based on violation of ARS 32-1854 (6), (20), (38), (42) & (45) and incorporate the reports by the Medical Consultant and the Investigator and specifically state that the Respondent committed ***ual conduct and contact with a patient, exposed chest piercings to a patient, admitted ***ual conduct with a patient, medical records were incomplete, prescribed ***s and benzodiazepines at the same time. Respondent told inconsistent statements under oath regarding having *** with a patient. Dr. Spiekerman seconded. ROLL CALL VOTE: The motion carried 5-0.
