Beware of Dr. Howard M Chasolen's Dental practice in Sarasota Fl. I went to him two years ago to have an upper dental screw in bridge unit made to connect to the wonderful implants and bone graft work that was done by Dr. Craig M Mish and his superb faculty of Sarasota fl. However, Dr Chasolen dramatically failed to the task of making what he had agreed to make and his promise that he could do it for the sum of $38,190.00 that was paid long before the work was ever sent out to a California discount dental lab. The lab failed to perform on over 10 attempts to make the unit functional and aesthetically appealing, as it was sent back and forth Approx. 15 times, 10 for the same corrections and 5 reg. times for the preliminary work. Dr. Chasolen and I agreed to do an entire new dental plan of a less costly glue in unit but for the same cost of the screw in unit, with a new local lab in Sarasota FL. I agreed to this to get the long and tedious nightmare over with, as I traveled 2500 miles and over 25 visits to Dr. Chasolen's office. But again, the so called new dental plan never took place as discussed on 3-14-18. Nor did I get a call back for an emergency call that I placed to Dr. Chasolen for the temp. unit that was falling out while talking and eating, and after over three days of waiting for a call back, I then had no choice but to go to a new dentist and paid a new fee of $27,000.00 for a new dental plan and a glue in unit ($11,000.00 cheaper than Dr. Chasolen's price. No charges were made to Dr. Chasolen for the failed unit that was cancelled and sent back to the California lab, but Dr. Chasolen thought it would be fair for him to keep all the money that I paid him, and he offered me a mere $7,500.00 refund from my $38,190.00 that I already gave to him plus my new expense of $27,000.00 so my total cost is now $65,5190.00. When I went to Dr. Chasolen for my dental plan I recently had a $7,000.00 new implant that was chipped by him while trying to remove the temp. bridge and also for the issue that the lab could not match it, so Dr Chasolen charged me $3,000.00 to remove the chipped crown. Now the new dentist charged me $2,200.00 to add that back into the new unit, I also paid $550.00 twice for a guide that was lost in Dr. Chasolen's possession

I would like to do another review for Dr. Howard Chasolen: Especially Of my serious mistake of going to Dr. Howard Chasolen in the first place,as I was given a referral to have the extensive ,serious and precision work that I needed, "BUT the referral was given to me as to see Dr. Chasolen" -and when I googled to find the address- I confusingly went to Dr. Howard Chasolen,not realizing that there was in fact a Dr.Ginger R Chasolen unbeknown to me as the google search just showed Dr.Howard Chasolen's address. However- When I did see Dr.Chasolen- I Questioned him as to the two names being the same -and if they are working together? He responded by saying they were married at one time -and that she should not be using his name in a profession manner- as was supposed to be from the final decree, so I assumed I was at the right address and with the right Dr. I still- "to date after making several requests and attempts to have him return the dental model,molds, and most importantly the failed screw-in inferior 12 unit bridge that he still has -But earlier in the game of the misleading events he stated not to worry that he was sending it back to the (Discounted ) lab located in California for a full refund- as they failed on the 10 attempts to make the unit -even though they kept on making the ten same mistakes over and over. Truth of The Matter is -that he no longer has it (but he told me on one occasion that he did not send it back and that he still has it trying to make me believe that the Lab kept all the money in which was part of the $38,190.00 that I paid to him long before the lab work was ever started. So in reality -the Lab got the shoddy work back and minus any of my money "as should be for their mistakes" So again learn from my epic mistake, that I unfortunately made and do not ever give 100% of the dental bill before the work is ever started ,and do diligence including where the lab is located, and please do not let 95% of the work ever be performed by an assistant "and to go on for 2 yrs." the assistant did do nice work -but my work needed full time by Dr.Chasolen -And shame on me for not doing as -the 5 dentist that I paid for on 5 different consultations- and that they could not do the work that I so desperately needed but said most importantly" in telling me to seek only a Prosthodontist to attempt the work that I needed" Dr. Misch that did all my extensive front bone graft and implants- stated to me that his wife is a Prosthodontist and that she could not take on such a endeavor, let alone a assistant do the 95% of my case that took 2 years and turned out to be a disaster and never any attempts to correct my issues, even them chipping a 1 yr. old molar crown,while being treated at Dr. Chasolen's office -and Dr. Chasolen having the gumption to charge me $3,000.00 to remove the chipped crown as he stated would make the lab wk.easier.I also paid $550.00 twice for a guide that was lost in Dr. Chasolen's possession. In short really I received "Sub-Treatment From My 2 yr.old nightmare"

:To Whom It May Concern: I was told by the advice from the VA- because of my substandard care that I received from Dr. Chasolen for the two years of a total nightmare, and being totally departed of my hard working sum of $38,000.00- and not having any of the work sent back to me- as I requested on several occasions- just to prove that it was in fact sent back to his discount dental lab in California for the total credit of any money that he paid to them- as he told me when he was sending it back - and telling the lab that it was a totally failing bridge unit- as he explained to me stating that we would receive any and all of the money that was paid to them. Dr. Chasolen then changed his story when he abandoned my treatment plan- that he again failed and could not put together the so called new plan with a new local lab to redo my work, he instead stated to me that the work was not sent back to the California lab- and as a result he was charged for the lab work of the failed unit, I then asked that he please send me all of the so called work that never was not sent back to the lab- and to please send it to me "to prove to myself that it was sent back to the lab and that he in fact did receive all of the money back" The dentist that finished my work even asked Dr. Chasolen for very important information pertaining to critical info. that was needed to complete my work with the new dentist but Dr. Chasolen never answered the request as it was obvious that a lot was being hidden from my substandard care that was provided with the treatment plan that I had for two years with Dr. Chasolen- and so very little was ever provided from Dr. Chasolen himself- other than the time and failed attempt that he made -trying to fix a chip that was done by his work on a $7,000.00 implant crown- even though i agreed to remove the chipped crown and let the lab have an easier time for the unit they were making, "low and behold Dr. Chasolen presented me with a $3500.00 bill to remove the crown that he had to remove because of the negligence in chipping it in the first place. To date my option that I am contemplating is to take out a reverse mortgage on my home to pursue a civil case- and try to get my $38,000.00 back and whatever merit of loss that a 80 year senior veteran would be entitled to for the two year night mare that I received while in treatment with Dr Chasolen. " Again- In Life We Learn From Mistakes, So Please Learn From Mine And Beware"

Follow-up After Talking With A Former Worker “ Dear Dr. Chasolen. As you know with all the discussions that we had, hopefully you can understand of my long and odious dental journey that you help put me on , and surly you knew that I had the expense of a total cost to me- starting from the university's cost for $14,000.00 plus Dr. Mish's cost of $11,800.00 and the $37,595.00 to your office ,and to date I have not a single red cent of receiving anything in workmanship nor any money back from you. plus the new cost of $27,000.00 for me to completely start all over again with another dentist, and yet another dentist to correct all of the mistakes at a cost of $29,000.00 so now with a grand total cost of $119,395.00 In all to me. I can not conceive how you must feel, of you departing an 81 year old veteran of $37,595.00 and stalling this case of my going back and forth to your office for two years, and aprox.30 visits at 80 miles per. visit -totaling over 2,500 miles and in very early morning travel (6:00 Am and on several return trips in heavy 5:00 Pm traffic as well) I think in all fairness, I was concerned of you having to pay a fee to the lab in California that you used. -and I offered to sign an affidavit to them in stating that the ten times that they could not get their work right, and that we were not going to accept the unit- as I didn't want them to charge you for the shoddy work that they were doing-time after time, and you said not to worry- as it's being sent back to them, and that they would then realize that you would not be charged at all for it, so instead you were able to keep all the money that I paid to you. I recently met a former worker of yours at the mall and I was told that all your office workers really felt bad for what you were putting me through- and how awful for what you did. I am asking at this time- in all fairness- again- if you would honor a financial adjustment for me ,as if I received a total completion of the work that you started- and if you didn't receive any money – I am sure it would not be fair to you- if you successfully completed my case, and not being paid, you would not like it. You did offer me any models , molds or anything that I may have needed for a new dentist, but I was told that dentist don't really want to use other dentist plans and case items. Please give me a call, an email- or perhaps to meet, and as I mentioned before "that I don't have sour g***s with you" and would be pleased to meet with you if you prefer. I would like to make mention of the $7,000.00 implant cost of Dr. Scott's implant crown- plus the initial cost for you to remove the crown ,as the lab could not line up their work with it, and in all fairness it was damaged at your office by your assistant that chipped it while trying to remove the temp. “Surprising for the two years of my treatments at your office ( you probably worked on my case for about 3 solid hours if any” And that is why it was a total failure, as you did not show any interest for the very complicated case that you stated -when you made me make payment of the $37,595.00 in full long before any work was ever done. You stated to me that it would be better if we removed that $7,000.00 crown to make the lab do their work with less effort, But you charged me $3,000.00 to remove the crown that was chipped at your neglect to removing the temp. unit correctly in the first place. and in closing I did spend an awful lot of unnecessary money- "and I thank God" for not having that additional cost when you sent me to your friend Dr. Ferris- who wanted a ridiculous amount of money for an unnecessary gum graft- and for the sum of $20,900.00- and Dr. Mish laughed and said that I did not need any such gum graft, and the cost to me from your friend Dr. Ferris to tell me of that,- was an additional charge to me for $200.00 Respectfully . My name is tammy bogart. I am so so angry as r many people in sarasota fl. Yhis is a serious problem. None or very few drivers speak english!!!!!!. What is goin on here. This is so dangerous for women like myself that need rides to emergency places such as hospitals. I need uber. But uber is unreliable. So sad upsetting and maddening. Pls alow us to select english speaking drivers or at least a driver that can speak enough english to understand simple directions. Thank u

I wish I could give 0. She was apparently high during my mother's surgery..opened her up and didn't complete the procedure. Left 30 feet of suture in her ankle and caused chronic osteomyelitis. She refused to show my mom her follow up x-rays and falsified her medical record. My mother is facing amputation due to Krista Howard's negligence. She should not be in charge of anyone's medical care....I would let a homeless person remove an ingrown toenail from my foot before I went to her

I'm not sure how Jessie represented my situation to you Dr Bruno, only you & Jessie are privy to that information. I called Dr Urbanski first, but because I hadn't had my 1st appointment with her & she didn't have my medical history to review, she not only couldn't prescribe pain meds, she couldn't even prescribe Tylenol for me. She urged me to call you & assured me you would prescribe something for my pain. NOT. As Jessie informed me, she has & I quote "BEEN" with you for 8 years & she has never known you to prescribe a pain med for any patient under your care. I only reqested 2 pain meds to get me thru the night until I had my appointment with Dr Urbanski the next morning. She rearranged her whole schedule the next day so she could treat me medically. I didn't need a lecture from Jessie about the opiate crises in this country. All she had to tell me was that Dr Bruno doesn't prescribe pain meds. I had another option, the ER. Skip the lecture from Jessie. Within an hour I would have been administered a pain med & had a script called into my pharmacy. Dr Bruno, you never call your own office as a patient, perhaps you should have your Mother, your Wife's Mother or an Aunt call & ask to talk to Jessie. I could write down pretty much my everything I said to Jessie. You could find out what their response to the way they were treated. Ultimately, you are responsible for the care & comfort of your patients.

Dr. Michael Sullivan, Gaylord, MI

Very sad experience. Dr. Sullivan performed a root canal on my number 13 molar. The root canals of this tooth are very narrow and extended. I warned the Dr. that I had a traumatic experience with elongated tooth roots which for teeth being extracted both as an adolescent and as a young serviceman in the Navy. Both of those times the dentists had a rough time with the teeth. The procedures lasted over 4 hours for the tooth when I was a child, and two visits over 3 hours each when I was in the Navy. Dr. Sullivan assured me that he was capable, and that his skill was superior. I believed him. After his first attempt at root canals on this tooth, Dr. Sullivan sent me home telling me that there wasn’t enough time that day to finish the procedure. Mind you I’m under the influence of sedation at this time. I told Dr. Sullivan that I would rather have the tooth pulled than go through another 3 hours in the chair. Amazingly I believed him again when he told me the reason he couldn’t finish was because of scheduling. He also said that he wanted to prove to his dental assistants he could finish the job! Being as high as I was on the anesthesia I made the poor judgement call to return. Two days went by with excruciating pain in my upper jaw. I was having to take Tylenol and Advil to relieve the discomfort. I went back to Sullivan’s office, asked AGAIN if it would be better to just extract the tooth, and he said it was his opinion he could save it, and this time he could finish the job and apply the cap. So I strapped on his gas mask and knocked myself stupid only to have the “Doctor” root around, stretch my jaw and break off half of what was left of the tooth. When I came to I distinctly remembered the snap, the dental assistant saying “Oh God!”, and the good Dr. scurrying away into another operatory. What was I told this time? Of course that I’d have to come back since it was the tooth’s fault it was so damaged that it cracked, and now he had to make a new cap with his machine. Stoned on the good Dr.’s laughing gas I told him “that’s okay, mistakes happen”. What an idiot I was. The laughing gas wore off and the Tylenol Advil routine lasted a week. I went back to Beacon Dental Center, this time for the last time, to finally get the remnants of my tooth capped. This time he squirted the glue in to fill the root canals, fixed the cap on the tooth, and away I went. Fast forward 5 years and I’m in the chair of a very competent dentist. The dentist asks if I’m in any pain and I mention the constant pain I’ve had on my left sinus area. I’ve felt this pain for three years. I’m shown the current X-ray of the area and the Dentist tells me that it’s not normal. The capped tooth is great work (machine made, remember) but there is stuff in the canals of Number 13 tooth that extend into the sinus. After visits to an Ear Nose Throat MD, Oral Surgeon and Endodontist specialists, I’m scheduled to have an operation to remove the so-called saved tooth, and resultant fistula created by the extensive overfilling of the Number 13 root canals with guttapurcha, the latex based root canal filling material. Predictably I could sue and win to recover the costs of these procedures, but the good Doctor has closed his practice, and is now enrolled in a school in Detroit. The lawyer I consulted doesn’t see that my case is strong enough to show the pain I’ve been living through since I trusted Dr. Sullivan. Of course there are risks to root canals. I accepted the possibility of a dry socket, and possible infection, but was re-assured by Sullivan that he’s done thousands of these procedures with no bad outcomes. General dentists can perform root canal procedures along with other dental procedures, but often they refer patients needing endodontic treatment to a specialized practice, who works in collaboration with your dentist. An endodontist is a dentist who specializes in saving teeth. Dr. Sullivan should have not touched that tooth after I warned him of the history of my other root issues, and certainly after he saw the roots narrowness and length.

This dentist is horrible, no bed side manner. I have ask her to look at a certain tooth every time I'm in there, and she just won't do it!! Tells me ”next time, we’ll have to do it another time at a check up” she's rude and condescending!! Stay clear if this witch!!
